PA POLST Featured in New Edition of Seniors Blue Book

The PA POLST (Physician Orders for Life-Sustaining Treatment) program, coordinated by the Jewish Healthcare Foundation, is featured in an article in the Hospice Care section of the Fall 2021–Winter 2022 Greater Pittsburgh Area Seniors Blue Book, a directory of resources related to aging well. The article describes the purpose of the POLST form, a medical order completed by a healthcare provider to document patients' wishes for care as their health declines. It also differentiates between advance directives and POLST. The digital version of the Blue Book is available here.
